ELECTRONIC SUBMISSION

1. Click blue "Download" Button below - interactive format of PDF will open up in a new window

2. Fill out all the required contact details directly on the downloaded form

3. Enter item number, description and quantity of item(s) you are seeking pricing on

4. Once all the necessary info has been entered, click the download button icon on the top right corner - be sure to save "with your changes"

5. Save file as Quote_Hospital-Name_Date - Ex) Quote_Universal-Hospital_3.1.22

6. Attach Quote Request with changes to an email and send to info@cwimed.com

PRINT/SCAN/FAX SUBMISSION

2. Click the print button icon in upper right hand of PDF window

3. Fill out required contact fields and list item number, description and quantity of item(s) you are seeking pricing on

4. Once all the necessary info has been filled in:

--Scan document to yourself (or take a picture) and attach file to an email and send it to info@cwimed.com

--Fax document to us at 1-508-790-1600
